below are the items required in order to participate in the SMM Athletic Program:
□ SPORTS PHYSICAL

□ CYO ATHLETES & PARENTS FORM – Must be signed by both student and
parent.
□ PAYMENT FOR PARTICIPATION FEE - $30.00 per student per sport. (cash or checks
payable to St. Mary Magdalen)
□ PAYMENT FOR ENTRANCE FEE - $20.00 per family per sport -doesn’t apply to St. MM
Tournament games or “away” games (cash or checks payable to St. Mary Magdalen)
□ STUDENT EMERGENCY FORM
□ VOLUNTEER HOURS REQUIRED – Read attached form to explain the
volunteer process.
UNIFORMS – No Charge – If uniform is not
returned when instructed, there will be a mandatory $50.00 replacement
charge (coaches will instruct when uniforms need to be returned)
